One corporation, multiple facilities

Community Health Services Corporation is a community owned organization whose nine Board of Directors consists of members from the local community. The organization is a nonprofit 501(c)3 corporation established many years ago to begin building LaBatisse on Dousman Apartments located at 700 South Dousman Street in Prairie du Chien, Wisconsin. In November 2004, Community Health Services Corporation formed two additional limited liability companies: Prairie Nursing Facility, LLC d/b/a Prairie Maison and Sannes Skogdalen Nursing Facility, LLC d/b/a Sannes Skogdalen Heim.

In order to fulfill a long-time strategic goal, Prairie du Chien Memorial Hospital Board of Directors approved an equity transfer of over six million dollars to both Prairie Maison and Sannes Skogdalen Heim to begin operations and establish their own entity under the direction of Community Health Services Corporation.

In addition in 2007, Prairie du Chien Memorial Hospital Board of Directors approved an equity transfer of $12 million to design and build a new senior village to be constructed in two Phases. Phase I includes a new 64-bed skilled nursing facility, 24 residential care apartments, 6 assisted living apartments, and a village plaza- city street area with shops. Phase I, Bluff Haven, is now complete and open for interested tenants and residents. Phase II is still in the design and planning stages, so look for more good things to come!
