Independent Living
Available at two seperate locations in Prairie du Chien, independent living is the right choice for those over 62 years of age and capable of managing their own medical conditions, but may require occasional intervention and/or monitoring.
Short-term rehabilitation
Short-term rehabilitation is available at both Prairie Maison at Bluff Haven in Prairie du Chien, and Sannes Skogdalen in Soldiers Grove. Ideal if you need some extra help recovering from surgery, illness or injury that requires hospitalization and you are not quite ready to go home. Recuperative care services including occupational, physical or speech therapy.
Assisted Living
Available at the Residence at Bluff Haven, this newly constructed facility offers independence and privacy to those 55 years of age or older who may require some extra assistance with meals, medications, daily tasks and health.
Skilled Nursing
Available at both Prairie Maison at Bluff Haven in Prairie du Chien and Sannes Skogdalen Heim in Soldiers Grove, skilled nursing is the right choice for those of any age who require extensive nursing and/or assistance with daily tasks.
