Nursing at Community Health Services embraces the Compassionate Relationship Centered Caring Model for our Care Delivery. This model of Care Delivery promotes emotional connections between residents, care givers and families.
We incorporate the six C’s of the model: Considerate, Compassionate, Courageous, Collaborative, Connecting and Compromising.
The Three Core Themes to the Relationship Centered Caring Model are…
1.) Know who I am and what matters.
2.) Understand how I feel about my experience.
3.) Work with me to shape the way things are done.
Caring Conversations with residents, care givers and families are core to premise of “The Resident Comes First”.
